We use PayPal Business to send financial statements and outstanding balance notifications. We will not include any clinician information in these communications. By signing below, you indicate your understanding that you may receive PayPal Business email notifications from Baker Mental Health.

This practice uses a scheduling system which is able to send appointment reminders via email or text. If you do not wish to participate in this system, please indicate such below.

 

Further, by engaging in service with BMHC you are indicating an understanding that AT NO TIME should clinical information be discussed through email or text message with your clinician. Email should ONLY be used for scheduling purposes or for general questions. Further, you are indicating that you understand that email, voicemail, and text messaging should never be used during an emergency.
