9. You have the right to participate in your own treatment planning and to give informed consent to recommended therapy, including medication.

11. You have the right to send and receive mail and to be provided with reasonable amounts of writing material and postage.

How To File a Complaint About Your Healthcare
Baker Mental Health Consulting, LLC (BMHC) wants you to receive the best mental health and substance abuse services possible. If you are not satisfied with your services in any way, we encourage you to provide us with that feedback.
